Account application

When you apply for an account, we need certain details in order to complete your application. These details need to comply with the following rules:

  • The telephone number: a number at your hospital or institution, not a private number
  • Email: email address at your hospital or institution. The address should be personal. A private or group email address cannot be used.
  • Function and hospital/institution: complete as fully as possible. These details are necessary in order to determine where entry should be granted

What happens with your details?

  • We use the details you have provided us with in order to determine which applications would be useful to you.
  • The details concerning your use of our website help us to develop and improve this further.
  • We send you information about new developments.
  • Your details are saved in a secure network
  • We do not pass on your details to third parties

Assigning a new account

  • Firstly, your application will be assessed. It will take 1-5 working days to complete this assessment. If you would like to register a patient sooner, or you need tissue urgently, please contact us by telephone:

  • After being assigned your account, you will be sent a login name and password by email

Instructions for use of account

  • Your account is personal; details about your login name and password should not be given to third parties
  • We use the Single Sign On (SSO) technique. SSO is 'once only login'. This mechanism means that verification of your user name and password only needs to occur once for different applications.
  • Never leave your workplace unsecured; block your entry if you leave your workplace. This is in order to protect against misuse by third parties.
  • To close your session, leave the SSO environment with LOG OUT. Afterwards,always close the browser (for example Internet Explorer or Firefox) completely.
  • Your password must be changed after 6 months. Instructions will appear automatically on screen.
  • Notification of changes to employment contract/function or hospital should be made to ETB-BISLIFE via info@etb-bislife.org
  • If you have forgotten your password you can apply for a new one by using the link on the login page. Having filled in your user name and email address, a new,temporary password will be sent to you automatically.
  • If your password is filled in incorrectly 5 times within 24 hours, your account will be blocked. This can only be unblocked by our network managers. In such cases, send an email to info@etb-bislife.org requesting your account be unblocked.
